The Renal DTPA Renogram offers a valuable graphical representation, charting the tracer’s dynamic journey through each kidney over time. This resulting graph, technically known as a renogram curve, characteristically displays three distinct phases:

Understanding the Renogram Phases

  1. Phase 1 (Perfusion/Vascular Phase):

    This initial phase shows a sharp upslope on the graph, typically lasting between 30-60 seconds. It directly reflects the arrival of the tracer-carrying blood at the kidney tissue. Notably, a blunted or delayed peak during this phase can suggest compromised blood flow, possibly indicating conditions like renal artery stenosis.

  2. Phase 2 (Uptake/Functional Phase):

    Following perfusion, the curve continues with a slower, steady rise over the next 1-5 minutes. This segment represents the active uptake of the DTPA tracer from the blood by the kidney’s filtering units (parenchyma). Therefore, the steepness or slope of this phase serves as a direct indicator of the glomerular filtration efficiency.

  3. Phase 3 (Excretion/Drainage Phase):

    Lastly, the curve shows a final downward slope. This visually depicts the tracer exiting the kidney’s collecting system and draining normally down into the bladder. Conversely, a significantly flattened, plateaued, or even rising curve during this phase is the classic graphical sign pointing towards an obstruction or impaired drainage.

Diagnostic Value

By meticulously analyzing the specific timing, shape, and relative intensity of these phases—and importantly, comparing the curves generated for the left and right kidneys—our experienced radiologists can provide a highly detailed, quantitative, and functional diagnosis. Indeed, this level of insight is often impossible to achieve solely through other imaging modalities like ultrasound or standard CT scans.
